Connect a Google Form to capture feedback

The dashboard is ready to route per-section feedback to a Google Form. You just need to create the Form and paste a few values into this HTML file.

1. Create a Google Form with these fields, in this order:
   1. Name — short answer
   2. Email — short answer
   3. Organisation — short answer
   4. NABH External Assessor — multiple choice with Yes / No
   5. Speciality — short answer
   6. Section — short answer
   7. Your feedback — paragraph
2. In the Form, click the ⋮ menu → Get pre-filled link.
3. Fill the literal placeholders NAME / EMAIL / ORG / Yes / SPEC / SECT into the matching fields, then click Get link and Copy link.
4. Open this HTML file in any text editor, find FEEDBACK_FORM near the top of the <script> block, and paste:
   • baseUrl — the part of the copied URL before the ?
   • entryIds.speciality, section, name, email, organisation, assessor — the matching entry.XXXX from the URL
5. Save the file and reload.

Once configured, the first time a reviewer clicks “Give feedback” the dashboard asks for their name, email, and organisation — those are saved in their browser so subsequent clicks open the form with all five fields prefilled, no retyping needed.
